Adaptation of navigated transcranial magnetic stimulation mappings to clinical use

Over the last five years transcranial magnetic stimulation with patient tailored navigation (nTMS) has become a routinely used non-invasive tool for mapping of functionally important cortical regions when planning surgical treatments for patients with brain tumor or severe epilepsy. While the reliability of the nTMS motor mappings have been established with healthy subjects and patients with brain tumors, the underlying cause for severe intractable epilepsy can range from developmental cortical malformation to injuries in early development, possibly giving rise to unusual cortical organization. The need for non-invasive cortical mapping of other than motor cortical region is needed, and the use of repetitive nTMS in speech-related brain area mappings have been suggested to be useful, although substantial variability and non-optimal replicability of the results have been reported.

Conventionally invasive cortical mapping is required for identification/confirmation of epileptogenic and/or eloquent cortical regions before resective surgery. For risk-benefit assessment and for up-front knowledge for surgery the extent of the nTMS maps and the existence of tissue margins between the eloquent cortical areas form lesions, is of key interest. In order to be able to make evidence based informed decisions, the limitations and precision of the clinically available nTMS mappings are needed.

My researc focus is on making the nesessary steps to establish the nTMS mappings as clinical routine tool for patients with epilepsy: describing the mapping protocol to be used in clinical motor cortex mappings and noting some important findings indicated by these mapping, determining the reliability of the mappings in patients with epilepsy, enlarging the clinical usability of the nTMS mappings to speech area mappings and implementing an automated routine to aid in the analysis. Also the error magnitude of the clinically used mappings is evaluated and compared with the state-of-the-art simulations.
